David Kessler (David Naughton) and Jack Goodman (Griffin Dunne) are American college students. They walk along a road in the lush Yorkshire pastures of Northern Britain. They arrive at a quaint hamlet. They saunter into the local tavern, the Butchered Sheep, where they are promptly greeted with warm hospitality by the patrons. One of the frequenters makes an inappropriate jest. As laughter ensues, including from David and Jack, Jack inquires about the peculiar altar adorned with candles and a pentacle. The atmosphere abruptly shifts to one of solemnity. The locals grow decidedly more morose. This prompts David and Jack to take their leave. Before their departure, the townsfolk cautioned them to remain on the path and to refrain from venturing into the fields.

As Jack and David meander along the thoroughfare, the heavens open, enveloping them in an ever-thickening mist. Thus, they unwittingly drift from the path, becoming mystified in the adjacent fields. Distantly, they perceive the ominous baying of an unseen creature, prompting Jack to suggest a retreat to the town. Then, they are startled by a menacing growl nearby, revealing an immense beast encircling them. They hastily attempt to flee, only to discover the creature has outflanked them. In their frantic escape, David stumbles and falls. As they chuckle at his misfortune and Raise lends assistance, Jack becomes the target of a ferocious wolf, which savagely rends him. Although David initially flees, he retraces his steps upon hearing Jack’s cries. He discovers Jack’s lifeless form, tragically mutilated, and finds himself pursued by the wolf, which mauls and claws at his face and shoulder. The sound of gunfire erupts; the villagers from the Butchered Sheep have arrived and slain the beast. As consciousness fades, David glimpses the bloodied remains of a naked man lying eerily nearby.

David awakens in a London clinic. His doctors and attendants reveal that he has been unconscious for a significant time since the attack. They also inform him that Jack is dead. The police have also informed his parents about his situation. David is devastated by the news of Jack’s murder, prompting the doctors to comfort him. Later, a police chief visits him. A representative from the American Department also visits. They question him about the circumstances of Jack’s death. David struggles to share much, only revealing that Jack was killed by a large, powerful creature. The officer tells David he has spoken with the patrons of the Butchered Sheep. They mentioned that Jack was murdered. The murder was committed by a recently escaped killer.

David begins to experience strange dreams that soon turn into nightmares; in one, he runs naked through the woods and comes upon a bed. There, he sees himself lying in it; when the bedridden David wakes up, he seems monstrous and growls. After a few days in the clinic, David encounters the ghost of Jack, who appears severely wounded. Jack has returned as a spirit to warn David that they are pursued by a ‘lycanthrope,’ a werewolf. The situation worsens. David is doomed to transform into a werewolf himself. This transformation will happen at the next full moon, just a few days away. Jack also insists that David must take his own life to break the werewolf’s bloodline, or he might attack others and perpetuate the cycle. Jack is cautious, believing that his trauma is affecting his mind. He assures David that the curse is real.

One of David’s medical caretakers, Alex Value (Jenny Agutter), is surprisingly taken with him and invites him to stay at her London flat after his discharge. They fall in love. Late one night while Alex sleeps, David wakes to use the bathroom. He’s shocked to find Jack, whose spirit has begun to decay. David still hesitates to accept Jack’s warning, even when Jack says he might hurt Alex. Jack disappears, leaving David to question his sanity.
Alone in Alex’s apartment while she works, David suddenly cries out in pain and gradually transforms into a wolf. Free in the city, he first stalks a man on the London Underground. The man hears David growling and quickly tries to escape, ultimately running as David chases him. He reaches an elevator and slips, and David kills him. In another part of the city, David attacks a group of homeless people and kills them all. He also drags a couple from their home and preys on another couple in the street, killing them too.

David stirs, believing he has had yet another disturbing nightmare, only to realize he is naked in the wolf’s enclosure at a zoo. He swipes a few balloons from a child and snatches a jacket from a woman. He rides back to Alex’s place on the Cylinder.
Meanwhile, David’s doctor, J. S. Hirsch (John Woodvine), travels to East Delegate. After being skeptical about David’s injuries upon his admission, Hirsch questions the locals to uncover why David’s wounds were thoroughly cleaned and dressed before being handed over to medical staff. The townspeople are reluctant to answer his inquiries; however, one of them speaks to the doctor outside the pub, revealing the truth and noting that David has cursed. The doctor returns to London and inquires about her new relationship with David, warning her to keep a close watch on him as she may be in grave danger.

Alex and David embark on a day out in London. During their outing, David spots a newspaper detailing the murders he committed and notes that things are becoming bizarre. Realizing Jack was right, he tries to turn himself in, even shouting anti-British remarks. Ultimately, overwhelmed, he flees, leaving Alex behind.

David finds himself in a seedy theater in Piccadilly Circus. Once again, Jack visits him, introducing him to the ghosts of his victims. They are furious with David, trying to sway him to take his own life immediately. Suddenly, David transforms back into the wolf and bursts out of the theater, attacking people on the streets and causing several horrific car crashes. The police chase him into a dead-end alley. Hirsch and Alex arrive, and she tries to calm David down to help him revert. With a glimmer of recognition in his eyes, David lunges at her, simulating an assault. The police shoot him dead in an instant.

Two American explorers are travelling through Europe and have stumbled upon a desolate part of the UK. Soon, the locals warn them about The Butchered Sheep and urge them to avoid the fields at night. Shortly after, they are attacked by a mysterious creature that kills one and injures the other. The locals kill the beast, and then the real fun begins. After regaining consciousness in the hospital, David meets Nurse Price and starts experiencing haunting dreams. His deceased friend Jack visits him. Jack tells him they were pursued by a werewolf. He also tells David he will soon transform into one himself. After leaving the hospital and moving in with Nurse Price, a series of horrifying murders occur. David realizes that his late friend might be telling the truth.
